The Hong Kong Marine Department has issued an information note regarding two fatal illnesses of malaria happened on two Hong Kong registered cargo ships last year in areas of Central Africa to draw the attention of operators to the lessons learnt from these incidents.

The Danish Maritime Accident Investigation Board has issued an accident report onboard Selandia Swan accident on July 23rd,2015. The findings from the investigation have indicated that, on that date, the 3rd officer was inspecting the lifebuoys mounted on the bridge wings while he was alone on watch on the bridge. During the inspection of the starboard lifebuoy, he accidentally lost his footing and fell overboard.

IMCA has issued Safety Flash for January 2016 including key safety matters and lessons learned from three diving-related incidents reported in late Q4 2015. One incident refers to aa pillar valve which forcefully parted from a High Pressure (HP) cylinder filled with 180 bar of compressed air. The incident occurred on board a DSV (Diving Support Vessel) whilst divers prepared for a dive and were putting on their diving suits. As a result of the failure, five divers were injured as the parted valve flew off the cylinder.

IMCA has issued Safety Flash for January 2016 including key safety matters and lessons learned from three diving-related incidents reported in late Q4 2015. One incident refers to a high potential near miss due to failure of both divers breathing air supply and dive stage recovery winch.

The vessel was port side at berth taking bunkers via a barge on the starboard side. The bunker barge was secured alongside the vessel and was initially overhanging the vessel’s stern by 10 to 12 metres. Although the berth immediately astern was vacant at the time, potentially it could have received another vessel. The bunker barge overhang would then become a potential hazard.

Two days before arriving in port, the Master received cargo documents from the shipper, which included an 11-page material safety data sheet (MSDS) on zinc concentrate. On receipt of the documents a safety meeting was held with the crew to discuss the cargo risks.

New Mars Report by The Nautical Institute refers to an incident involving bauxite liquefaction and cautions always to have the cargo tested for moisture content and to not transport it if moisture is above the transportable moisture limit.
