Power failure leads to grounding

Electronic charts are a boon for navigators - but so are bridge windows. Latest Mars Report refers to an incident in which a Master seduced by the screen found that the electronic picture can be misleading. The ship was about to enter a lock when it suffered a voltage drop and the engine-room team initiated blackout procedure. The Master put the engine astern to abort the lock entry and began a 180° starboard turn to head south away from land.

UK MAIB issues its Safety Digest 1/2016

The UK MAIB has announced the publication of the first issue for 2016 of its Safety Digest which includes lessons learned from maritime accidents. This latest edition of the Safety Digest contains 25 articles which are examples of poor risk awareness demonstrated by the crews of vessels. The publication once again highlights that a cautionary approach should be second nature to every mariner about to start a task or embark on a course of action on the bridge, in the engine room or on deck.

Slip/Trip resulting in Lost Time Injury

IMCA has reported an incident in which a crewman slipped on deck and fell, resulting in a broken arm. The incident occurred when the chief engineer was returning to the engine room from the bridge via the accommodation. In spite of instructions to stop personnel going on deck during heavy weather, the chief engineer decided to leave the accommodation in the winch housing area to return to the machinery spaces.
