In its latest Safety Scenario, the Swedish Club presents an accident where a stevedore lost his life after falling from a ladder. The stevedore was climbing up the ladder using only one hand as he had a tea cup in the other, which could not fit in his boiler suit pocket, and he was not wearing a safety harness as well.

The ATSB issued an investigation report on the fire onboard the general cargo ship 'BBC Xingang', while berthed at Mayfield number four berth in Newcastle, New South Wales, in December 2017. The incident highlights that, prior to hot work, cargo coverings should be carefully assessed and adequate protection should be provided.

In its latest Safety Digest, UK MAIB describes a near-miss of a Ro-Ro vessel. The ship was trying to berth but it encountered problems because at that day there was fog and sunlight in the port, which resulted in a significant glare. As the ship was performing manoeuvres to berth, the master warned the passengers and the crew to prepare for impact, which was averted, but for only 3 metres.

On the occasion of the investigation on the flooding and sinking of the fishing vessel 'Ocean Way' in March 2017, the UK MAIB issued a safety message to the fishing industry about developing and practising potential coping strategies in an emergency such as major flooding.

In its latest Safety Flashes for May, IMCA presents an incident of a slip onboard that resulted in injury. The incident highlighted the importance of identifying and marking all the potentially hazardous spots, so that these kind of accidents to be prevented.

The NTSB issued an illustrated digest report, summarizing key safety issues and recommendations, on the El Faro sinking, the deadliest shipping disaster involving a US-flagged vessel in more than 30 years. Earlier investigations have attributed the disaster to the captain’s failure, as well as to inefficient procedures compliance onboard. 

Although it is a new risk, cyber-crime is considered one of the top five risks in shipping.  This is where we stand at the moment; we know only four paths to implementation in shipping. But do we know how successful are they or even which is the most effective?

DMAIB released its report about an unintended release of a rescue boat on 6 December 2017, in Norway. The accident happened as an annual survey of the ship, "BERGENS-FJORD", was about to take place. As part of the survey, the crew had to lower the rescue boat to sea level according to the ship’s procedures.

UK MAIB describes a pilot ladder incident in its latest Safety Digest. Three engineering technicians were ready to disembark a bulk carrier, using a pilot ladder. When two of the technicians were on the ladder, it failed and the two of them fell from a respectable height. Both were transferred to hospital, as they had sustained injuries.

In its latest Safety Digest, UK MAIB deals with an incident involving monoxide poisoning. The owner of a small motor cruiser was trying to pump out water off of the engine compartment bilge. However, a little later a friend called the owner, without answer. Shortly after, paramedics found the owner unconscious, he was transferred to hospital, but never recovered consciousness.
