The UK MAIB issued a safety warning informing that a defective throw bag rescue line was discovered while Warrington Rowing Club was conducting boat capsize drills for new rowers at Halton Baths in Cheshire. A 15m long polypropylene rescue line in a throw bag parted while a young person in the water was being pulled to the side of the pool during a simulated rescue. 

The UK Health & Safety Executive issued a report about a company that has been fined after an employee got injured when he was hit by a steel plate that fell from a crane, on 10 December 2015. The company has been fined £32,000 and ordered to pay costs of £3444.80 and a victim surcharge of £120.00.

The International Association of Oil & Gas Producers published an alert of a failure of strain-hardened Type 304 stainless steel bolts. The failure happened at an offshore facility during a pneumatic leak test of gas piping exiting a test separator. The failure has been attributed to chloride-induced stress corrosion cracking.

In the latest Safety Digest, UK MAIB presents the case of a small fire on a Ro-Ro ship. Repairs were planned during an evening passage, involving hot work inside the boiler. As the discharge of passengers and vehicles had started, the chief engineer informed the safety officer that a small fire had broke out in the boiler.

Following Mirai, WannaCry and NotPetya attacks that caused substantial damages across many sectors in the past, safeguarding the ONG sector has been proved of foremost importance. Specifically, a cyber-attack in the ONG may lead to plant shutdown, equipment damage, undetected spills or safety measures violation that may result in injuries and even death. After several high-profile attacks, including the 2012 Saudi Aramco attack which unleashed a virus that affected 30,000 workstations, ONG companies have established their own protocols and prevention procedures.

In June 2017, A.P. Moller - Maersk fell victim to a major cyber-attack caused by the NotPetya malware, which also affected many organisations globally. As a result, Maersk’s operations in transport and logistics businesses were disrupted, leading to unwarranted impact.

In its latest Safety Scenario, the Swedish Club presents an accident where a stevedore lost his life after falling from a ladder. The stevedore was climbing up the ladder using only one hand as he had a tea cup in the other, which could not fit in his boiler suit pocket, and he was not wearing a safety harness as well.

The ATSB issued an investigation report on the fire onboard the general cargo ship 'BBC Xingang', while berthed at Mayfield number four berth in Newcastle, New South Wales, in December 2017. The incident highlights that, prior to hot work, cargo coverings should be carefully assessed and adequate protection should be provided.

In its latest Safety Digest, UK MAIB describes a near-miss of a Ro-Ro vessel. The ship was trying to berth but it encountered problems because at that day there was fog and sunlight in the port, which resulted in a significant glare. As the ship was performing manoeuvres to berth, the master warned the passengers and the crew to prepare for impact, which was averted, but for only 3 metres.

On the occasion of the investigation on the flooding and sinking of the fishing vessel 'Ocean Way' in March 2017, the UK MAIB issued a safety message to the fishing industry about developing and practising potential coping strategies in an emergency such as major flooding.
