According to IMCA, two articulated lorries with 20’ trailers were organised to move equipment, including lifeboat davits, a power-pack, winches and sheave blocks, from one location to another. However, the load of one of the trailers feel as it was inadequately secured. The equipment did not sustain any other serious damages.

IMCA reported an incident where an intermediate bulk container was dropped from a crane to deck. The incident happened when a vessel crane was being used with a certified lifting device to lift and move an IBC containing 1000kg of monoethylene glycol. The IBC fell about 3.5 metres to the deck and burst open, spilling the contents.

IMCA reported two near miss incidents involving potential dropped objects. The objects were left on top of a temporary buoyancy module during the installation of a tension leg platform. In the first incident, two subsea lamps were on top of the TBM and in the second a rigging crew reported a wire cable retaining pin lying loose on top a TBM.

IMCA informs about a riser that moved unexpectedly away from the vessel. The incident occurred during the deployment of a production riser from a hang-off frame located starboard aft. IMCA said that the causes of the incident were a lack of risk perception and the failure to follow procedures.

MSF has published two safety alerts relating to potential dropped objects from cargo operations. In the first incident, the crew found a twist lock pocket cover missing from the aft main deck and in the second, the crew on a platform noticed an object on top of a container onboard the supply vessel. 

In its latest Safety Digest, UK MAIB describes a case of a fatal injury of a banksman who was acting as a banksman for the loading of an unaccompanied piece of freight that was being loaded by a tug-master. The crew member became trapped between the rear of the trailer and a vent housing. 

The Marine Safety Forum informed about a near miss when during routine operations using a platform crane to transfer a bulk hose between a platform and a platform supply vessel, a worker got caught in the rigging and was lifted off of the deck by the crane. The AB did not sustain any injuries from this incident.

The Transportation Safety Board of Canada issued an investigation report on the grounding of articulated tug-barge composed of the tug' Nathan E. Stewart' and the tank barge 'DBL 55', resulting in the barge's sinking and an oil discharge into the water. The report indicated fatigue as the key cause of the accident. 

UK MAIB analyzed the flooding of a fishing vessel while underway and the rescue of its two crew by an RNLI lifeboat. The investigation revealed that the weight and location of the fish catch can significantly affect the vessel's stability. The fishermen were uninjured and the actions of the RNLI crew prevented the potter sinking.

In its latest Safety Digest, UK MAIB describes an incident where a fishing vessel caught fire. Flames broke out in the wheelhouse through a cable penetration in the deck, while an explosion blew out the wheelhouse windows and blew off the roof. It is possible that a leak allowed petrol into the vessel’s bilge .
