In its latest Safety Digest, UK MAIB describes a case of a fatal injury of a banksman who was acting as a banksman for the loading of an unaccompanied piece of freight that was being loaded by a tug-master. The crew member became trapped between the rear of the trailer and a vent housing. 

The Marine Safety Forum informed about a near miss when during routine operations using a platform crane to transfer a bulk hose between a platform and a platform supply vessel, a worker got caught in the rigging and was lifted off of the deck by the crane. The AB did not sustain any injuries from this incident.

The Transportation Safety Board of Canada issued an investigation report on the grounding of articulated tug-barge composed of the tug' Nathan E. Stewart' and the tank barge 'DBL 55', resulting in the barge's sinking and an oil discharge into the water. The report indicated fatigue as the key cause of the accident. 

UK MAIB analyzed the flooding of a fishing vessel while underway and the rescue of its two crew by an RNLI lifeboat. The investigation revealed that the weight and location of the fish catch can significantly affect the vessel's stability. The fishermen were uninjured and the actions of the RNLI crew prevented the potter sinking.

In its latest Safety Digest, UK MAIB describes an incident where a fishing vessel caught fire. Flames broke out in the wheelhouse through a cable penetration in the deck, while an explosion blew out the wheelhouse windows and blew off the roof. It is possible that a leak allowed petrol into the vessel’s bilge .

The UK MAIB issued a safety warning informing that a defective throw bag rescue line was discovered while Warrington Rowing Club was conducting boat capsize drills for new rowers at Halton Baths in Cheshire. A 15m long polypropylene rescue line in a throw bag parted while a young person in the water was being pulled to the side of the pool during a simulated rescue. 

The UK Health & Safety Executive issued a report about a company that has been fined after an employee got injured when he was hit by a steel plate that fell from a crane, on 10 December 2015. The company has been fined £32,000 and ordered to pay costs of £3444.80 and a victim surcharge of £120.00.

The International Association of Oil & Gas Producers published an alert of a failure of strain-hardened Type 304 stainless steel bolts. The failure happened at an offshore facility during a pneumatic leak test of gas piping exiting a test separator. The failure has been attributed to chloride-induced stress corrosion cracking.

In the latest Safety Digest, UK MAIB presents the case of a small fire on a Ro-Ro ship. Repairs were planned during an evening passage, involving hot work inside the boiler. As the discharge of passengers and vehicles had started, the chief engineer informed the safety officer that a small fire had broke out in the boiler.

Following Mirai, WannaCry and NotPetya attacks that caused substantial damages across many sectors in the past, safeguarding the ONG sector has been proved of foremost importance. Specifically, a cyber-attack in the ONG may lead to plant shutdown, equipment damage, undetected spills or safety measures violation that may result in injuries and even death. After several high-profile attacks, including the 2012 Saudi Aramco attack which unleashed a virus that affected 30,000 workstations, ONG companies have established their own protocols and prevention procedures.
