NTSB investigation: Overheating of electrical wiring led to Grand Sun fire

NTSB issued an investigation report on the explosion and fire onboard the offshore supply vessel ‘Grand Sun’ while in the Gulf of Mexico, in October 2018. The report identified overheating of electrical wiring, associated with a chest freezer or the receptacle powering it, as key cause.

Princess of the Orient: Erroneous maneuvering leads to deadly sinking

The Philippines has a long history of fatal maritime accidents. This time, SAFETY4SEA focuses on the deadly sinking of the ‘Princess of the Orient’. The ferry sank off Fortune Island, near the provinces of Cavite and Batangas, on September 18, 1998, claiming the lives of 150 people. Extreme weather conditions, along with ‘’erroneous maneuvering of the vessel by the captain’’, were the immediate causes of the accident.

In its latest edition of Safety Digest, the UK MAIB focuses on a fishing vessel’s accident, in which its personnel accidentally got engaged in shooting creels. While the vessel was facing rough weather conditions, the personnel couldn’t use the a self-shooting system, leading to the engaging of three deckhands in the shooting creels.
