Oil spill incidents seem to occur more nowadays. Thinking of major oil spills, there were three major ones that took place in the US, starting from Santa Barbara in 1969, Exxon Valden in 1989 and Deepwater Horizon in 2010.
IMCA informed of a near-miss incident during drilling operations on a land-based drilling rig, when the thimble within the hard eye of the winch wire momentarily caught (snagged) on the edge of the drill rig chassis. It then released in an uncontrolled manner very close to the driller.
The Petroleum Safety Authority Norway (PSA) completed its investigation on the fire in the mud laboratory on West Phoenix of 5 November 2018. The report identified several breaches of the regulations. This rig is operated by Seadrill Management (Seadrill), and was drilling well 6406/2-9 S in Norwegian Sea with Equinor as the operator, when the incident took place.
Norway’s Petroleum Safety Authority completed the investigation of the movement-compensated walkway incident on the Tambar field in July 2018, revealing several breaches of safety regulations. The Authority noted that Tambar is the only facility in operation in Norway, where an attempt has been made to land a walkway from a ship.
Through its SafeOCS program, the US BSEE released its annual report Blowout Prevention System Safety, informing that through 2017, 18 of 25 operators associated with rig operations in the Gulf of Mexico reported 1,129 equipment component failure events, which represents a decrease from 2016.
