Following investigation of a 2019 California dive boat fire that killed 34 people, the National Transportation Safety Board Tuesday called for major safety improvements to small passenger vessels, including interconnected smoke detectors and a USCG inspection program. 

The South African Maritime Safety Authority (SAMSA) commented on a recent incident that took place on the Vaal Dam, commencing a preliminary investigation to understand the causes and factors that led to the tragic incident.

A recent incident investigation concerning the sinking of the vessel Bourbon Rhode on 26 September 2019 in the Atlantic Ocean, revealed a safety issue which is of great importance.

The UK MAIB launched a new investigation report concerning the container feeder vessel Thea II that after an engine failure it had to anchor, but dragged its anchor towards the Binks Shoal. Both vessels grounded.

The US Coast Guard continues to investigate the loss of 21 containers from the barge Ho Omaka Hou and monitor response operations in Hawaii. The tug Hoku Loa reported loss of cargo from the barge as they transited from Honolulu to Hilo Harbor in the early morning of 22 June. 

During routine sea chest strainer cleaning at sea on board a Hong Kong registered container ship, a heavy sea chest cover suddenly falling from height and fatally hit the fitter working underneath.

The Hong Kong Marine Department issued marine notice to draw the attention to ship operators to lessons learned from a collision between a fishing vessel and an oil tanker in Dangan Channel.

The collision highlights once again the need for more efforts toward improving Bridge Resource Management and addressing best practices for enhanced safety of navigation

A series of causes contributed to vessel grounding; the safety investigation revealed that the course recommended by the pilot was different from the one prepared in the passage planning. Moreover, during navigation in El Dekheila Channel (Egypt), the vessel navigated close to shallow areas, and subsequently ran aground and remained stranded.
