Prosecutors raid Coast Guard offices concerning Sewol case

A new special prosecution unit raided multiple offices of the Korea Coast Guard with the aim to find evidence in regards to the sinking of the ferry Sewol in 2014, which claimed the lives of 304 people. The raids are said to be part of the latest in a long series of investigations into the loss of the ferry Sewol.  It was reported that the raids recovered personnel records listing the names of the first responders on the day of the sinking, as well as other documents related to the accident. 

Investigation of Snorre B fire finds breaches of regulations

Norway’s Petroleum Safety Authority identified breaches of regulations in relation to a fire incident on the Snorre B facility on 1 May 2019. The investigation revealed that iron sulphide, which caused the fire, was not known to be present in the separator, so no appropriate measures were taken. 

Missing US navy sailor identified as deceased

The US Department of Defense announced Tuesday the death of a sailor who has been missing since mid July and is believed to have gone overboard from the US warship ‘Abraham Lincoln’, while supporting Operation Freedom’s Sentinel.

RMI investigates fatality caused by failure of maintenance platform

The Republic of the Marshall Islands (RMI) Maritime Administrator informed that it is currently conducting a marine safety investigation of a fatality that occurred when the extension of the lower maintenance platform for one of the ship’s provision cranes detached and fell 8-9 meters. 

USCG begins investigation on Auger fatalities

The US Coast Guard announced it has initiated a formal investigation on the fatal incident that occurred onboard Shell’s offshore production platform ‘Auger’, on 30 June 2019. A Shell employee and a contractor with oilfield services provider Danos Inc, were killed during a routine test of a lifeboat launch.

USCG begins investigation into three tugboats sinking

Shortly after completing pollution response, the US Coast Guard announced it has begun an investigation on a major marine casualty involving three vessels sinking in the Illinois River near Hardin, on 5 July 2019. The vessels contained approximately 4,850 gallons of diesel fuel and oil.

ATSB investigates collision with tree incident on Sweets Lagoon

Two passengers were injured, one of them seriously, after the airboat ‘Gale Force’ collided with a tree, during tourism operations on Sweets Lagoon, Northern Territory, at about 1010 hours on 22 June 2019. The ATSB has commenced an investigation in the collision.
