According to local media, an unknown poisonous gas leaked from a container at port Karachi in Pakistan, killing at least five people, while about 70 others fainted. Pakistan's Federal Minister for Maritime Affairs Ali Zaidi commented the cause of the gas leak is not known yet, adding that a high-level team has been formed to investigate the matter.

Fire took place at the SPD9 platform, in the South Pars gas field, on June 12. About 14 people were working at the platform the moment of the fire, and after the explosion they were transported to Asaluyeh, Southern Province of Bushehr, by helicopter. No casualties were reported, while the fire has now been contained.

The US Coast Guard informed that it is monitoring a natural gas and condensate leak from an orphan well, located four nautical miles southeast of Bob Hall Pier near Corpus Christi, Texas, on Thursday, May 9. The US Coast Guard is cooperating with the Railroad Commission of Texas, Texas General Land Office and Texas Commission on Environmental Quality to monitor and secure the release.

The production of Equinor's Aasta Hasteen gas field continues being out of operation as the Norwegian authorities and Equinor investigate the cause of a gas leak, which occurred on the field the previous week. No personal injuries have been reported as a consequence of the incident. According to Equinor, the operator company, personnel were in the immediate vicinity when the leak occurred.

The PSA Norway has concluded its investigation of the LNG leak in Equinor’s Hammerfest LNG plant at Melkøya on 17 June 2018. Several breaches of the regulations have been identified. PSA identified two non-conformities, as well as three improvement points. Equinor has until 24 January 2019 to explain how the non-conformities will be dealt.

According to Equinor an LPG leakage took place at one of the Mongstad facilities, on 10 December. The unit with the leak was shut down, while depressurizing operations were conducted. A little later the same day, the leakage had stopped and the situation was normalised.

Petroleum Safety Authority Norway informed that it started an investigation into an LNG leak at the Hammerfest LNG plant on 17 June 2018. The incident happened while a road tanker was being  filled with LNG at the Equinor facility at Melkøya outside Hammerfest in northern Norway.

Two crewmembers of the bulk carrier Fen Hui Hai lost their lives, while another one is hospitalized after inhaling toxic gas. The incident happened on Sunday, 22 of April, in India. The incident happened as one of the crew went into the hold and was affected by gas, and two others went in the hold to help him, also suffering gas poisoning.

Norway's Petroleum Safety Authority has given a notice of order to French Engie to take the necessary measures to ensure that safety equipment on its offshore plants is properly operated, tested and maintained, following the condensate leak on the Gjøa field in June.

On 13 January 2017, the Antigua & Barbuda registered dry cargo vessel Nortrader suffered two explosions in quick succession while anchored in Plymouth Sound, England. The vessel was loaded with a cargo of 2333 tonnes of unprocessed incinerator bottom ash.
