Two shipyard workers have been killed An explosion at a shipyard in Singapore occurred on Thursday afternoon, killing at least two people, authorities said.The Civil Defense Force said it received a call about the incident at about 1:20 p.m. in the afternoon, adding that the incident involved a barge undergoing maintenance work at the shipyard.No further details were available at the moment.Local broadcaster Channel NewsAsia quoted witnesses as saying that glass panels were broken at her office across the road. It also caused a ceiling panel to collapse but no one was injured.The witness, who is identified only as Mandy, said the ground shook when the explosion occurred.Another eye witness, Adrian Lee, said he saw people running out after the blast caused a roof to collapse. Report on the investigation of a fire and explosion on board a bulk carrier

Hotwork not performed in accordance with established safety procedures The UK Marine Accident Investigation Branch (MAIB) issued the report of its investigation of the fire and explosion on board a self-unloading (SUL) bulk carrier in western Scotland on 2 July 2010.The fire probably was the result of hotwork recently performed near the cargo conveyor belt. The hotwork was not performed in accordance with established safety procedures. Elements of the conveyor belt are highly flammable.There were no effective means of early detection or containment of the fire, which spread to the engineroom and nearby spaces used for chemical storage. Storage vessel M/T Alpsray exploded

The crew of 5 was saved The German inland tank m/v "Alpsray", 1125 ts (ENI-No.: 04008570; built 1954) exploded in the night of Mar 29 at the BP refinery Lingen. The ship had 900 tons super benzol on board.The vessel sank after various explosions at a water depth of 3,5 metres.The accident happended during loading works. Large amounts of oil leaked out.
