Collision and explosion kills nine

The Nautical Institute has issued Mars Report referring to a fatal accident in which several elements, each of them unremarkable, combined to create a disastrous collision and explosion at sea. Distraction on the bridge, lack of clarity about intentions, an unjustified assumption and excessive speed all played their part.

Two container ships collide off Chinese coast

A Maersk Line ship entitled the 'Safmarine Meru' collided with the Northen Jasper, a German-owned container vessel while en route from Qingdao to Ningbo in China. The Maersk Line's 292-meter vessel was severely damaged and caught fire. The ship was abandoned shortly after; the crew of both vessel was reported to be safe. The collision took place in open seas 138 miles east of the Chinese port of Ningbo, south of Shanghai.

BSU report on the collision between cargo vessel and lock gate at Brunsbüttel

The Cuprus-flagged general cargo ship Saint George was en route from Amsterdam to Lubeck via the Kiel Canal when she reached Brunsbuttel at about midday on 20 March 2015. Apart from the master and pilot, the bridge was manned by the third officer and a helmsman for the entry into the Neue Nordschleuse lock. The ship sailed into the lock chamber at low speed with engine stopped. When the Saint George was supposed to be brought to a halt in the lock, she accelerated unexpectedly.
