Insufficient risk management led cargo ship to contact wharf

ATSB published its final report into the cargo ship 'Madang Coast' impacting a wharf at the Port of Townsville. The risk management processes were not sufficiently mature nor resilient enough to identify and mitigate risks in pilotage services. The incident has led to the updating and full implementation of a safety management system, in order to better identify risks and how to mitigate them.

Watch: Salvage team detonates torpedoes from sunken Helge Ingstad

Although the delay was long concerning weather conditions, the salvage process for the sunken Norwegian frigate Helge Ingstad is almost completed, and the Norwegian Navy expects to raise it on February 7 for transport to a nearby naval base. Salvage divers managed to remove the majority of the frigate's missiles and torpedoes for disposal. As the torpedoes were too sensitive to be transferred, they were destroyed nearby.

Watch: Vessels break free of moorings due to heavy rainfall and ice flow

The city of Troy in New York published a video depicting several unmanned boats and vessels pass beneath the Troy-Menands Bridge on the Hudson River. The vessels broke free of their moorings on Friday, January 25, because of heavy rainfall and ice flow on the Hudson River. One of them, a river cruise boat, broke its moorings and drifted downstream, hitting a bridge and wedging itself under another in Albany.

Watch: Two ferries collide at Olbia, Italy

This video depicts a collision between the ferry Cruise Olbia, with the ferry Athara at Olbia port, Sardinia, Italy, on January 24. The collision was minor, but it caused damage to both ships. The collision took place probably due to rough weather. Athara's bridge wing was damaged and remained at port, while Cruise Olbia continued her voyage after leaving Olbia harbor.
