The skipper of a ceremonial waka, which capsized on Whakatane River bar in January last year, has been convicted and fined $1,350 by a New Zealand court, for putting his five crew in unnecessary danger. Maritime NZ said this is a reminder to the skippers that they are responsible for the safety of everybody on-board.

On the night between 21 and 22 December 2016, the Danish offshore supply ships MÆRSK SEARCHER and MÆRSK SHIPPER capsized and sank in the Bay of Biscay, off the French coast, while being towed by another offshore supply ship, MÆRSK BATTLER, en route to Turkey.

The tug Domingue girted and capsized while assisting the container ship CMA CGM Simba departing from the port of Tulear, Madagascar, on September 2016. As a result, two of Domingue’s five crew died. UK MAIB published an investigation report on the accident, providing significant safety issues. 

On Friday, 12 May, the Italian highest court upheld the sentence of the former Costa Concordia captain to 16 years in jail, for his role to the cruise ship capsize, that caused death of 32 people off Tuscan, in 2012.

The UK MAIB investigates a case in which isolated working led to girting during mooring operations and highlights the importance of the appropriate training of the crew, in order to prevent such cases. Girting means the risk of capsizing due to high athwart ships towing forces.

AMSA has issued the ongoing investigation on a commercial fishing vessel,Bowen, that capsized approximately 4 kilometres off Bowen in Queensland with three crew on board, on May 25. Queensland Police coordinated a search and rescue response.
