Australian Transport Safety Bureau released an investigation report focusing on a fire incident onboard a bulk carrier, that took five days to contain and extinguish.

The 5,780 TEU 'APL England' was rolling and pitching heavily in rough seas and high winds when it lost 50 containers overboard about 46 nautical miles south-east of Sydney on 24 May 2020, ATSB said in its preliminary investigation report. 

The Panamanian-flagged bulk carrier Bulk India ran aground in 2018 in a channel during its departure from Dampier, Western Australia when it lost steering and propulsion due to a total electrical blackout from a failed generator. The ATSB issued an investigation report on the incident.

ATSB issued a report on a crew fatality in the elevator trunk on the container ship OOCL Kuala Lumpur, in June 2018. While driving the elevator from the cage top, the electro-technical officer became trapped between the moving cage and the bulkhead, and was fatally injured.

Australia's Transport Safety Bureau (ATSB) issued an investigation report on the grounding of the Australian Border Force cutter Roebuck Bay (ABFC Roebuck Bay) on Henry Reef, Queensland, in September 2017. The report identified an amendment on the passage plan which led the vessel to the reef and poor use of ECDIS.
