The Marine Safety Forum has issued safety alert regarding a serious injury incident during maintenance on deck.
The incident:
The vessels AB’s discovered a roller on a winch that was stuck. They decided to try to loosen it by the use of the rail crane fitted on the vessel. When the crane driver tried to pull it lose the pallet strap hook broke off the crane hook and hitting the AB in the back.
The AB was severely injured. The pallet strop hook was incorrectly secured to the crane hook making it much weaker than the limits of the lifting strops.
Causes:
1. Incorrect use of tool/equipment
2. Personnel standing in snapback zone
3. Lack of Planning and Risk Assessing Task
Recommendations:
1. Proper planning and risk assessment of all work tasks to ensure safe operation
2. Ensure correct tool is used for the task
