Lack of training in the operation and maintenance of the ship’s OBA set
The Australian Transport Safety Bureau (ATSB) issued the report of its investigation into the serious crew member injury on board a bulk carrier at sea off Eden, New South Wales on 11 October 2011.
Investigation revealed that the crew member suffered burns when the air compressor he was using to fill an oxygen breathing apparatus (OBA) cylinder exploded. The explosion occurred when oil from the compressor ignited in the hot oxygen-rich environment.
The crew had not been trained in the operation and maintenance of the ship’s OBA set.
